HHOI QIP Co-Design Team The HHOI QIP Co-Design Team consists of a core group of stakeholders including primary care team members and patients from practices across the state, payers, practice coaches, and content experts. This team will develop multi-level strategies toimprove cardiovascular health and reduce disparities in the ABCS (Aspirin when indicated, Blood pressure, Cholesterol management, and Smoking) with a primary focus on blood pressure and smoking. |

HHOI QIP Implementation Team The HHOI QIP Implementation Team, oversees the implementation of the Heart Healthy QI intervention. This team will assist in developing resources used for the external QI infrastructure, oversee the practice facilitator training, and oversee QI and clinical content development for the QI project with other key team members. |

HHOI QI External Support Infrastructure Team The HHOI QI External Support Infrastructure Team builds off the infrastructure from the 7 academic medical centers and three regional health improvement collaboratives across the state to enhance sustainability of Cardi-OH. By expanding Cardi-OH to more cohesively integrate the 3 regional health improvement collaboratives quality improvement efforts and infrastructure, we will be poised to assist with additional statewide quality improvement efforts as prioritized by Cardi-OH. |
