Our Heart Healthy Ohio Initiative collaborative leverages and seeks to expand Cardi-OH’s existing statewide collaborative (www.cardi-oh.org) by adding partners with quality improvement expertise to develop an external QI support infrastructure for the state and assist in the heart healthy quality improvement project. In addition, we expand to include additional payers and professional organizations to grow our network.
