Our Heart Healthy Quality Improvement Project is funded by the Agency for Healthcare Research and Quality to improve cardiovascular health and reduce disparities in cardiovascular health (the #1 cause of death in Ohio).
Starting in May 2022, 50 primary care practice sites will implement evidence-based strategies to improve cardiovascular health and reduce disparities in the ABCS (Aspirin when indicated, Blood pressure, Cholesterol management, and Smoking) with a primary focus on blood pressure and smoking. Continuing education credits and Maintenance of Certification available.
